I have done a portfolio before in english 111 and 112 class. We would write papers periodically through out the semester and at the end of the semester we would have to write one page of how we think we did for our over all portfolio and our strengths and weaknesses. We would put all the papers together in a folder and the people in the english department would grade us on our overall performance of the portfolio. This is would be the final word on whether you passed the class or not. They would bring it back to you and it would say pass or no pass on the overall folder. I think this is the only portfolios I have done. I do not remember if I have done other portfolios before college.
